Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards:    (a) On or about December 23, 2013, an employee was exposed to and struck by a hazard when, among other things, he entered an active work environment on the Rig 329 floor where ice and oil-based mud built up around the locking mechanism of the top drive elevator links on Rig 329 and prevented the locking mechanism from working properly and caused the elevators to rotate at a rate of speed higher than what was set in the drive parameters.    Abatement Note: Among other methods, one feasible and acceptable means of abatement to correct the hazard would be to follow Tesco's operating procedure which states, "Before operating the pipe handler, ensure the links and elevators are hanging vertically."
