The employer did not perform a hazard assessment to determine if safety hazards were present and what personal protective equipment would be utilized to reduce or eliminate those employee safety hazards.  This assessment would include:      a. Select and have each affected employee use the types of PPE that will protect the employee from hazards identified,   b. Communicate selection decisions to each employee, and    c. Select PPE that properly fits each affected employee.
