29 CFR 1926.451(b)(10): Scaffold components manufactured by different manufacturers were intermixed such that the components fit together with force and the scaffold's structural integrity was not maintained by the user:      a) On or about January 9, 2014 and at times prior, employees were exposed to fall injuries while working on a scaffold.  The scaffolds were constructed with intermixed different manufacturer's components that did not fit together. Three-plank frame scaffolds were intermixed five-plank frames.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:      a) On or about January 9, 2014 and at times prior, employees were exposed to serious fall injuries while accessing the scaffolds using their crossbrace. The scaffolds were not provided with a ladder for the employees' access to the scaffold.

The second level of scaffolds, in height of 16 feet, was not installed with guard rails on the side to prevent the employees from falls to the lower level.
