29 CFR 1926.451(b)(2): Scaffold platforms and walkways were not at least 18 inches wide: On or about January 13, 2014, an employee performing framing activities on a residential home was working from a job made scaffold with a platform that was not 18 inches in width, exposing the employee to a fall hazard approximately 25 feet to the deck below.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: On or about January 13, 2014, employees performing framing activities such as but not limited to installing roof ridge and corner beams on a new residential home were using a job built scaffold system and exposed to an unprotected fall approximately 25 feet to the lower decked floor below.
