29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:     (a) Third floor, new townhouse construction located at 655 Green Crest Lane, Milton, GA 30004: Employees were exposed to fall hazards of approximately 21-feet, 9-inches to the lower level while working on the third floor work surface snapping chalk lines with no fall protection system in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    (a) Front side, new townhouse construction located at 655 Green Crest Lane, Milton, GA 30004. Employees were exposed to fall hazards up to 21-feet, 9-inches while accessing the third floor working surface with a 24' Werner (Model D1324-2) Extension Ladder which was not extended 3 feet past the landing surface. Employees used an adjacent top plate to gain access from the ladder to the third story landing surface as the ladder provided was not long enough to reach the intended work surface.
