(a) Front side, new townhouse construction located on Lot#288 Green Crest Lane, Milton, GA 30004: Employees were exposed to falls while using a 1/2-inch x 4-foot x 8-foot OSB plywood sheet and a pneumatic hose as a means to access the second floor working level located approximately 9-feet, 6-inches from the top of the plywood pile and 11-feet, 1-inch from the ground level.
