29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds shall be fully planked or decked between the front uprights and the guardrail supports.  a.  At the construction site located at Building #4 on Ellington Circle, Columbus, Ohio, an employee was installing window flashing from an approximately twelve foot tall scaffold that was not fully planked exposing the employee to a fall hazard.  b.  At the construction site located at Building #3 on Ellington Circle, Columbus, Ohio, employees were installing stucco from the second level, approximately twelve feet, of a five level scaffold that was not fully planked exposing the employee to a fall hazard.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level.    a.  At the construction site located at Building #4 on Ellington Circle, Columbus, Ohio, an employee was installing window flashing from the top platform of an approximately twelve foot tall tubular scaffold without any form of fall protection, exposing the employee to a fall hazard.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews.  a.  At the construction site located at Building #3 on Ellington Circle, Columbus, Ohio, employees were performing stucco work on a new three story apartment building from the second level of a five level scaffold without guardrails installed, thereby exposing employees to a fall hazard.  b.  At the construction site located at Building #3 on Ellington Circle, Columbus, Ohio, an employee was performing stucco work on a new three story apartment building from the third level of a five level scaffold without guardrails installed, thereby exposing employees to a fall hazard.
