OSH ACT of 1970 Section (5)(a)(1):     Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee(s) in that employee(s) were exposed to the hazard of caught-in:    Inside of the Bridgeport Room, employee(s) were exposed to an amputation/laceration hazard due to a possible machine process malfunction on the following equipment:    a/  Bridgeport Milling Machine (M-2).    b/  Bridgeport Milling Machine (M-7).    One method of abatement to correct this apparent hazard is to install emergency stop devices as required by ANSI B11.8-2001, paragraph 6.12.1.    DISCLAIMER:    a/  The employer is not limited to the abatement method(s) suggested by OSHA.  b/  The employer is responsible for selecting and implementing an effective abatement method.

29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toe boards:  Note:  Toe boards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard:    a/ In the Main shop, an employee was setting up a part on the Gray Horizontal Boring Mill  (6 inch diameter spindle) while standing on an unguarded platform that measured approximately 49 inches from the floor.  Conventional fall protection methods were not in place.       IN THE ALTERNATIVE:    29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, and maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    a/ In the Main shop, an employee was setting up a part on the Gray horizontal Boring Mill  (6 inch diameter spindle) while standing on an unguarded platform that measured approximately 49 inches from the floor.
