On or about January 13, 2014 - at the above addressed jobsite, an employee performing roofing operations on the south side of the house, approximately twenty (20) feet above the ground was not protected from falls, thereby exposing the employee to fall hazards.

29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. a. On or about January 13, 2014 - at the above addressed jobsite, an employee was performing roofing operations from a ladder jack scaffold that had a platform extending greater than eighteen (>18") inches past its supports, thereby exposing the employee to a fall of approximately eighteen (18) feet seven (7) inches to the ground. Crossbraces shall not be used as a means of access. a. On or about January 13, 2014 - at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold located on the south side of the building, approximately eighteen (18) feet seven (7) inches above the ground was not provided with an access ladder, thereby exposing the employee to fall hazards. b. On or about January 13, 2014 - at the above addressed jobsite, an employee conducting roofing operations from a ladder jack scaffold located on the west side of the building approximately nineteen (19) feet above the ground was not provided with an access ladder, thereby exposing the employee to fall hazards.
