No guardrails were present on the east side of the wall/scaffold.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: a) For the construction site on U.S. 301 in Summertree Plaza, Zephyrhills, FL, as observed on or about 01/14/2014, employees were working off of a scaffold platform elevated via the forks of a rough terrain forklift. The platform consisted of a piece of plywood, was not attached to the forks of the forklift, had no guardrails, and the forklift was not designed to be used with the platform.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a) For the construction site on of U.S. 301 in Summertree Plaza, Zephyrhills, FL, as observed on or about 01/14/2014: extension cords used by employees with power tools had their outer insulation pulled away at the fitting resulting in lack of strain relief for the inner terminal conductors.
