29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing hazards):     (a)  The employer failed to conduct an inspection at 980 Elm Road in Cedarbug, Wisconsin, to ensure all safety precautions were implemented prior to the commencement of work.  This expose employees to fall hazards.

29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    (a) The employer failed to ensure that employees working on a ladder jack scaffold were protected from falling by the use of fall protection.  This exposed employees to a fall hazard of ~17 feet.

29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii):     (a) The employer did not ensure that all employees, which could potentially be exposed to fall hazards, were provided with training on the use, limitations, and proper implementation of fall protection systems.  This exposed employees to fall hazards.
