Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) South-east Roof - On or about January 8, 2014 an employee was exposed to a fall hazard while working on roof of a building 22 feet above concrete with no guardrail system, personal fall arrest system or safety net system. (b) South-west Roof - On or about January 8, 2014 an employee was exposed to a fall hazard while working on roof of a building 22 feet above concrete with no guardrail system, personal fall arrest system or safety net system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards. The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: (a) Construction Site - On or about January 8, 2014 employees were not trained on how to recognize, prevent and minimize exposure to fall hazards.
