29 CFR 1910.29(a)(3)(v): The work level platform of scaffolds (towers) of wood, aluminum, or plywood planking, steel or expanded metal, were not the full width of the scaffold, except for necessary openings. 29 CFR 1910.29(a)(3): At a height of approximately 16 feet, the work platform did not cover the entire width of the scaffold. The work platform consisted of only two of the required three pre-fabricated planks. Employees were exposed to falls through the scaffold.

29 CFR 1910.29(a)(3)(vii): Work levels 10 feet or higher above the ground or floor did not have a guardrail of 2" x 4" nominal or the equivalent, installed no less than 36" or more than 42" high, with a mid-rail of 1"x 4" nominal lumber or equivalent: Two employees worked from a mobile scaffold at heights up to 16 feet which was not equipped with standard guardrails around the entire work platform. Employees were exposed to falls to the exterior of the scaffold.

29 CFR 1910.333(b)(2): While an employee was exposed to contact with parts of fixed electric equipment or circuits which were deenergized, the circuits energizing the parts were not locked out or tagged, or both, in accordance with the requirements of this paragraph (b) of 29 CFR 1910.333(b)(2): The circuit energizing the parking lot lights two employees were working on was not locked-out after the circuit was deenergized. The employees were exposed to sudden reenergization of the circuit.
