29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. (a) Roof, new residential construction located at 424 Wade Farm Drive, Powder Springs, GA 30127: Employee was exposed to potential eye injuries while operating a Hitachi (Model NR83AB2-S1) Framing Nail Gun without eye protection. The employee installing 1/2-inch OSB plywood with 2-3/8-inch plastic collated nails with compressed air set at 120 PSI was not provided with safety glasses or goggles while operating these tools.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Roof, new residential construction located at 424 Wade Farm Drive, Powder Springs, GA 30127: Employee was exposed to falls while installing 1/2-inch OSB sheathing on a 12/12 pitched roof at eave heights of approximately 18-feet, 6-inches with no fall protection system in place. (b) Second floor, new residential construction located at 424 Wade Farm Drive, Powder Springs, GA 30127: Employee was exposed to falls of approximately 11-feet, 5-inches while cutting plywood on the second floor work surface without a fall protection system in place.
