29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about June 14, 2013, and at times prior thereto, employees at the facility administered hazardous drugs and they were not provided and did not use chemotherapy gloves. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 902994, citation number 2, item number 1 and was affirmed as a final order on 1/17/2014, with respect to a workplace located at Southern Oregon Rehabilitation Center & Clinics. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 912239, citation number 1, item number 1 and was affirmed as a final order on 10/11/2013, with respect to a workplace located at Eugene CBOC. The Veterans Health Administration was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 312394661, citation2 number 1, item number 1 and was affirmed as a final order on 10/13/2011, with respect to a workplace located at the Eugene CBOC.
