29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about January 14, 2014, at a work site located at 8506 U.S. 377, Benbrook, Texas: an employee worked from a vehicle-mounted rotating work platform without fall protection was exposed to a fall of approximately 20 feet.
