On or about January 14, 2014, on the south side of the residential construction site, employees engaged in framing activities were not provided hardhats while working below other employees and were exposed to struck-by hazards from falling objects.

On or about January 14, 2014, on the south side of the residential home, employees utilized an extension ladder to access the second level and the side rails did not extend 3 feet above the landing exposing the employees to a fall hazard of approximately 10 feet to the ground below.
