An abatement demand letter was sent to the employer on July 3, 2014.  Abatement documentation is required for this item.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:       a) Southwest corner of building, on or about 1-14-14:  Employees were operating a hand held circular saw that was not protected by a ground fault circuit interrupter (GFCI).

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:       a) Southwest corner of building, main roof, on or about 1-14-14:  Employees were accessing the roof via a single section of extension ladder that was extended 17 inches above the landing surface, exposing the employees to a fall hazard of 22 feet.       b) West side of building, porch roof, on or about 1-14-14:  Employees were accessing the porch roof via a single section of extension  ladder that was extended 15 inches above the landing surface, exposing the employees to a fall hazard of  10 feet 10 inches.
