29 CFR 1926.153(h)(16): Storage of containers awaiting use was not in accordance with paragraphs (j) and (k) of this section:    a)  At the jobsite, inside the building, in the garage, on or about 1/13/14:  A 100 lb propane gas cylinder was stored, upright, without restraint from falling, for at least one week.      b)  At the jobsite, inside the building, in the basement area, on or about 1/13/14:  A 100 lb propane gas cylinder was stored, upright, without restraiint from falling, for at least one week.    c)  At the jobsite, outside the building, next to the garage doors, on or about 1/13/14: 3 100 lb propane gas cylinders were stored, upright, without restraint from falling, for at least one week.      Abatement Certification must be submitted for this item.

29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken:  a)  At the jobsite, outside the building, next to the garage doors, on or about 1/13/14: 3 100 lb propane gas cylinders were stored, upright, without restraint from falling, without protection from vehicle traffic, for at least one week.    Abatement Certification must be submitted for this item.

29 CFR 1926.404(b)(1)(ii): 29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:   a)  On the jobsite, inside the garage, on or about 1/13/14:  Employees were using a DeWALT table saw and a DeWALT radial saw, both powered from an outlet that was not GFI protected.

29 CFR 1926.405(g)(2)(iv): The employer shall ensure that flexible cords are connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a)  At the jobsite, in the garage, on or about 1/13/14:  Employees were using power tools connected to a quad outlet where the cord was pulled free from the housing.  b)  At the jobsite, in the garage, on or about 1/13/14:  Employees were using a DeWALT radial saw where the power connection was pulled free from the saw.  c)  At the jobsite, in the basement, on or about 1/13/14:  Employees were working in close proximity to power cords that were pulled free from the plugs.  Those cords were used to provide lighting.    d)  At the jobsite, on the main floor, on or about 1/13/14:  Employees were working in close proximity to power cords that were pulled free from the plugs.  Those cords were used to provide lighting and power hand tools.
