29 CFR 1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. (a) On or about January 14, 2014, in the Vehicle Maintenance Shop in Building 798, no method of machine guardinging was installed on the Northern Industrial Floor Drill-press potentially exposing employees whom utilized the drill press to ingoing nip point and rotating parts hazards. (b) On or about January 22, 2014, in the Metal Technology Shop in Building 758, no method of machine guardinging was installed on the Rockwell Drill Press, Serial Number CR 4SC17D192FECCW, potentially exposing employees whom utilized the drill press to ingoing nip point and rotating parts hazards.

29 CFR 1910.215(d)(3): The surface of the abarasive wheel was observed with foreign materials build-up. All contact surfaces of wheels, blotters and flanges shall be flat and free of foreign matter. On or about January 23, 2014, at the Jet Propulsion/ Engine shop, in Buidling 756, there was a build-up,of foreign on the surface the grinding wheel. This potentially exposed employees to struck-by hazards.

29 CFR 1910.1001(j)(7)(iv): The employer did not provide training to employees who perform housekeeping operations in an area which contains ACM or PACM, an asbestos awareness training course. On or about January 14, 2014, the Indiana Air National Guard did not provide asbestos awareness training to employees whom perform housekeeping or response to fiber release episode clean-up. Employees were potentially exposed to cancer and lung disease hazards.

29 CFR 1910.179(k)(2): Rated load test. Test loads shall not be more than 125 percent of the rated load unless otherwise recommended by the manufacturer. The test reports shall be placed on file where readily available to appointed personnel. (a) On or about January 22, 2014, at the Aerospace Ground Equpment Shop, in Building 768, no rated load test report was provided for Crane GOHH. (b) On or about January 23 at the Jet Propulsion/Engine Shop, Building 756, no rated load test report was provided for the overhead electric hoist, GZ-118.
