29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:   (a) Box Room Area - - Employees used the Dayton 42672C Bench Grinder to dress rods for the injection molding machine.  The work rests were not adjusted to a maximum 1/8 inch.  The work rest for the left wheel was set to 3/8 inch and the work rest for the right wheel was set to 3/8 inch, on or about January 13, 2014.   (b) Screw Machine Area - - Employees used the Delta 234239 W 6082 Bench Grinder to dress cutting tools for the CNC machine.  The work rests were not adjusted to a maximum 1/8 inch, on or about January 13, 2014.   (c) Screw Machine Area - - Employees used the Landis Machine Company # 298A-2 Grinder to dress cutting tools for the CNC machine.  The work rests were not adjusted to a maximum 1/8 inch.  The work rest for the wheel was not installed, on or about January 13, 2014.   No abatement certification or documentation required.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   (a) Box Room Area - Employees used the Dayton 42672C Bench Grinder to dress rods for the injection molding machine.  There was no tongue guard installed, thus exposing employees to injuries, on or about January 13, 2014.    (b) Screw Machine Area - Employees used the Delta 234239 W 6082 Bench Grinder to dress cutting tools for the CNC machine.  There was no tongue guard installed, thus exposing employees to injuries, on or about January 13, 2014.    (c) Screw Machine Area - Employees used the Landis Machine Company # 298A-2 Grinder to dress cutting tools for the CNC machine. 29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):  (a) Volume Control Department - An employee used an unguarded Rousselle Mechanical Press, serial number 18439, to manufacture parts used in a downstream operation.  The employee was exposed to crushing and amputation hazards from the lack of point of operation guarding, on or about October 16, 2013.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.219(d)(1): Pulleys with parts seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   (a) Voume Control Department - An employee operated the Rousselle Punch Press to manufacture parts used down stream in production.  There was no guard for the drive belt and pulley, which were 5 feet 9 inches above the floor.  Employees were exposed to amputation hazards from the in running nip point of the pulley, on or about October 16, 2013.   No abatement certification or documentation required.
