Employee placing the 6 inch water line and valve in the trench was exposed to struck-by and crushing injuries from a cave-in, in the up to 5 foot, 7 inch deep trench which was not adequately sloped or protected by shields or shoring.                                            Violation observed on or about 12/20/2013.  The C. Freitas Plumbing & Heating Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 313063851, citation number 1, item number 1b and was affirmed as a final order on May 4, 2009, with respect to a workplace located at 403 Kearny Avenue, Kearny, NJ.
