29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, and rotating parts:  a)  In the middle of the Shop, the Wright 2 headed drill (Model # HDC386) had an unguarded rotating shaft. b)  In the Warehouse, three offset presses ( AB Dick 9970, 9970 and 9970D) used daily had unguarded rotating rollers and ingoing nip points. c)  In the Finishing area, the Lawson Pace Maker II (Lawson 56 cutter SN # BD2019) had an unguarded clamp.

29 CFR 1910.1200(f)(6)(ii)  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  a)  In the Screen area, a safety can of isopropyl alcohol used daily was not labeled with identity or hazard warning. b)  Near the Wash area, a safety can of isopropyl alcohol used daily was not labeled with identity or hazard warning. c)  In the Ink Mix area, a safety can of isopropyl alcohol used daily was not labeled with identity or hazard warning.  d)  In the Wash Out Area, a 55-gallon white plastic drum was not labeled with identity or hazard warning.

29 CFR 1910.1200(h)(3)(iv)   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  At the facility, employees who used chemicals including but not limted to Isopropyl alcohol and On-Press Cleaner with Citrus, were not trained on the new Global Harmonization System (GHS) labeling and safety data sheets.
