29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a. On or about January 13, 2014, in the excavation, four employees were not wearing hard hats, exposing themselves to being struck by chunks of clay, rocks, wood and bricks.

29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:    a. On or about January 8, 2014, on a 25 X 17 trench box, an employee was unhooking crane slings without a fall protection system, exposing himself to a 10 feet fall hazard.     b. On or about January 8, 2014, on quay wall, two employees were using blowers to remove debris without a fall protection system, exposing themselves to a 10-foot 5-inch fall hazard.

On or about January 13, 2014, inside of a 15-foot deep excavation, five employees were not protected from a collapse hazard by use of an adequate protective system.
