29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing or blocking of machines or equipment from energy sources: a. Instances existed where press blocks, which were designed to be attached to an interlock switch by a chain thereby ensuring interlock function when the block was put in place, were not attached to the chain. The blocks were required to be used during some set-up procedures/changeovers. In addition the two parts of the interlock switches were designed to be kept closed by a clip that was not present on some of the switches. Abatement documentation is required for this violation.
