Instances existed where press blocks, which were designed to be attached to an interlock switch by a chain thereby ensuring interlock function when the block was put in place, were not attached to the chain.  The blocks were required to be used during some set-up procedures/changeovers.  In addition the two parts of the interlock switches were designed to be kept closed by a clip that was not present on some of the switches.  Abatement documentation is required for this violation.
