On or about January 15, 2013, on the north side of the construction site, employees engaged in roofing activities utilized an extension cord with the outer sheathing split exposing internal wiring.  Employees were exposed to electrical hazards.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use.    On or about January 15, 2013, on the north side of the construction site, employees engaged in metal roofing activities, used a work platform that was not approved by the forklift manufacturer, exposing the employees to a fall hazard of approximately 16 feet.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied.   On or about January 15, 2013, on the north side of the construction site, employees engaged in metal roofing activities, worked from a work platform that was not securely attached to the forks of a Case 586E forklift that was moved while occupied, exposing the employees to a fall hazard of approximately 16 feet.

On or about January 15, 2013, on the north side of the construction site, employees who worked from a work platform without mid-rails installed on all sides were exposed to a fall hazard of approximately 15 feet while engaged in metal roofing activities.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.    On or about January 15, 2013, on the north side of the construction site, employees who worked without fall protection were exposed to a fall hazard of approximately 16 feet while engaged in metal roofing activities.
