29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use. On or about January 15, 2013, on the north side of the construction site, employees engaged in metal roofing activities, used a work platform that was not approved by the forklift manufacturer, exposing the employees to a fall hazard of approximately 16 feet.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied. On or about January 15, 2013, on the north side of the construction site, employees engaged in metal roofing activities, worked from a work platform that was not securely attached to the forks of a Case 586E forklift that was moved while occupied, exposing the employees to a fall hazard of approximately 16 feet.
