On or about November 1, 2013, at the warehouse area, an employee was exposed to a caught by hazard while cutting wood with a chopsaw  (i.e. Dewalt, Model Number D2825, Serial Number 095170) that lacked a guard.

On or about January 14, 2014, at the warehouse area of the above addressed worksite, employees were exposed to a struck by hazard while driving a powered industrial truck (i.e. Yale, Serial No. E187VI56084) that had one of its back indicator lights broken.
