29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a.   On or about November 1, 2013, at the warehouse area, an employee was exposed to a caught by hazard while cutting wood with a chopsaw  (i.e. Dewalt, Model Number D2825, Serial Number 095170) that lacked a guard.

On or about January 14, 2014, at the assembly area of the above addressed worksite, an employee was exposed to an electrical hazard while soldering cables with an iron that lacked a continuous path to ground.

On or about January 14, 2014, at the warehouse area of the above addressed worksite, employees were exposed to a struck by hazard while driving a powered industrial truck (i.e. Yale, Serial No. E187VI56084) that had one of its back indicator lights broken.
