On or about January 22, 2014, at Building #3 of the above addressed jobsite, an employee was exposed to the hazard of falling 5'-1" to a lower level while working standing on the platform of cracker mill #1.  The platform lacked a midrail on one of the sections of the side facing east and on the side facing north.    c.  On or about January 22, 2014, at Building #3 of the above addressed jobsite, an employee was exposed to the hazard of falling 5'-2" to a lower level while working standing on the platform of screening #3.  The platform lacked a midrail.    Liberty Tire Recycling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 947985, citation number 1, item number 2 and was affirmed as a final order on December 18, 2013, with respect to a workplace located at 2058 Hwy 14S, Saltillo, MS 38866.
