29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):     a.  On or about January 15, 2014, at Building #2 of the above addressed jobsite, employees were exposed to a struck by hazard while a powered industrial truck (i.e.Toyota, Serial No. 61236) was operated with a non-functioning back up alarm and front lights.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a.  On or about January 15, 2014, at Building #2 of the above addressed jobsite, employees were exposed to a caught by hazard while working by an in-going nip point at the end conveyor coming out of Rasper #3.     b.  On or about January 15, 2014, at Building #2 of the above addressed jobsite, employees were exposed to caught by hazard while working by an in-going nip point at the beginning of the discharge conveyor from Rasper #3 to the tire wire area.

29 CFR 1910.23(c)(1):  Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      a.  On or about January 22, 2014, at Building #3 of the above addressed jobsite, an employee was exposed to the hazard of falling 4'-11" to a lower level while working standing on the platform of cracker mill #2.  The platform lacked a midrail on the side facing north, and lacked a guardrail on two sections of the side facing east.    b.  On or about January 22, 2014, at Building #3 of the above addressed jobsite, an employee was exposed to the hazard of falling 5'-1" to a lower level while working standing on the platform of cracker mill #1.  The platform lacked a midrail on one of the sections of the side facing east and on the side facing north.    c.  On or about January 22, 2014, at Building #3 of the above addressed jobsite, an employee was exposed to the hazard of falling 5'-2" to a lower level while working standing on the platform of screening #3.  The platform lacked a midrail.    Liberty Tire Recycling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 947985, citation number 1, item number 2 and was affirmed as a final order on December 18, 2013, with respect to a workplace located at 2058 Hwy 14S, Saltillo, MS 38866.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft:      a.  On or about January 15, 2014, at Building #2 of the above addressed jobsite, employees were exposed to a caught by hazard while working by an unguarded shaft (i.e. 1.25-inch diameter, 3-inch long) at the end of the discharge conveyor from Rasper #3 to the tire wire area.    b.  On or about January 15, 2014, at Building #2 of the above addressed jobsite, employees were exposed to a caught by hazard while working by an unguarded shaft (i.e. 1-inch diameter, 2-inch long) at the beginning of conveyor to wire baler.    c.  On or about January 15, 2014, at Building #2 of the above addressed jobsite, employees were exposed to a caught by hazard while working by an unguarded shaft (i.e. 1.25-inch diameter, 2-inch long) at the beginning of conveyor to baler #1.    Liberty Tire Recycling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 947985, citation number 1, item number 5 and was affirmed as a final order on December 18, 2013, with respect to a workplace located at 2058 Hwy 14S, Saltillo, MS 38866.
