On the west wall, behind the packaging area, there was a 110/220 volt panel box with four (4) unused openings located inside of the breaker box. Employees were exposed to contact with live parts.

29 CFR 1910.303(f): Each service, feeder and branch circuit, at its disconnecting means or over current device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:(a) (LOCATION) (IDENTIFY EQUIPMENT) (STATE HAZARD(S) TO EMPLOYEE(S)) 1. Inside of the break area, on the south wall, there was a 110/220 volt panel box that was not marked to indicate the purpose of each breaker contained inside of the box. There was no circuit directory inside of the breaker box. 2. Inside of the file storage room, on the west wall, there was a 110/220 volt panel box with no circuit directory (ledger)located inside of the box. 3. On the west wall, behind the packaging area, there was a large 110/220 volt panel box with no circuit directory (ledger) located inside of the breaker box. 4. On the west wall, behind the packaging area, there was a small 110/220 volt panel box with no circuit directory (ledger) located inside of the panel box.
