All energy isolating devices that are needed to control the energy to the machine or equipment shall be physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s).      a) On or about March 14, 2014,  electrical disconnect on the Electric Furnace in the mold department was not actuated to disable power while positioning the deck pot and changing out the furnace liner, when multiple employees were working on the liner change out.

The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.        a) The employer did not develop procedure, implement training, machine specific, annual review, procedures to take in the event that the actual lock applicator was not present and when it was multiple employees involved in locking out,  with respect to the use of locking out in preventing release of unexpected energy during the servicing and maintenance including but not limited to the Electric Furnace and Mold Machines.
