29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:    a) Walkway over fuel tanks, on or about 01/15/2014; Employees walking the catwalk above the fuel storage tanks were exposed to fall hazards of approximately 10 feet while checking for fuel levels. There was no mid rail installed to prevent employees from falling to the ground below.

29 CFR 1910.24(h): Standard railings were not provided on the open sides of all exposed stairways and stair platforms:  a) Stairway to access fuel tanks, on or about 01/15/2014; Employees climbing the stairways without stair rails to access the catwalk above the fuel storage tanks were exposed to fall hazards while accessing the catwalk platform to check for fuel levels.

29 CFR 1910.106(f)(3)(iii): Valves used for the final control for filling tank vehicles were not manually held open or equipped with automatic means for shutting off the flow when the vehicle is full or after filling of a preset amount:      a) Loading Station closest to Harris Road, on or about 01/15/2014;  The fuel arm, designed to be manually held open was tied open by a rope.  The automatic shut off was not designed to engage at a preset amount or when full, but instead was set to shut off at 30 minutes.
