29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Bindery by Auto Feeds Cutter:  Employees were exposed to electrical hazards as a metal electrical duplex outlet box with prepunched knockouts was not used or installed in accordance with listing or labelling instructions as it was used with flexible cord and was left loose on the floor. A metal duplex electrical box with prepunched knockouts is required to be mounted to a wall or a piece of equipment with conduit.
