29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. On or about November 13, 2013, the employer did not ensure two employees installing sheet metal on to the roof deck with unprotected sides and edges were in use of fall protection exposing the employees to a fall of approximately 32 feet to the concrete below. Weststar Builders, Inc. was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.760 (a)(1), which was contained in OSHA inspection numbers 314931536, citation number 01, item number 004 and was affirmed as a final order on 05/19/2011, with respect to a workplace located at 100 N. Kansas, Weatherford, Oklahoma 73096 and 314935545, citation number 01, item number 006, and was affirmed as final order on 01/30/2012, with respect to a workplace located at 811 SW 19th Street, Moore, Oklahoma 73160.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from falling or flying objects were not protected by protective helmets. On or about November 13, 2013, the employer did not ensure employees working below areas where employees were working were in use of head protection exposing the employees to a struck-by hazard.
