29 CFR 1910.134(g)(1)(i)(A):   The employer permitted respirators with tight-fitting face-pieces to be worn by employees who had facial hair that came between the sealing surface of the face-piece and the face or that interfered with the valve function:  On or about 01/15/2014, an employee who performs finishing operations while wearing 3M - L7608 Half-Face Respirator, had facial hair that can result in an improper seal of the respirator, exposing him to hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  On or about 01/15/2014, first floor production area, the distance of the tongue guard to the abrasive wheel belonging to a Double Head Verticle Grinding Machine, illegible model/serial numbers, was approximately one inch.

In the finishing department, where employees used chemicals such as, but not limited to, Wilsonart 900 Series Adhesive, Lokweld 10- White Glue Wood Adhesive, and DG-1 Laquer Thinner, training on the new label elements and safety data sheets formats was not given by the effective date of December 1, 2013.
