Facility yard - On or about January 15, 2014, employees who operated a wood grinder system were required to access the following open sided elevated platforms:    1) CAT engine platform  2) Sorting station platform (east)    The elevated platforms were not provided with a standard guard railing on all open sides thereby exposing employees to hazards associated with falls.

Facility yard - On or about January 15, 2014, an employee was required to operate the CAT engine of a wood grinder system on an open sided platform.  The open sided platform was adjacent to an inclined moving conveyor without a standard guard railing thereby exposing employees to fall hazards.

29 CFR 1910.27(f): "Maintenance." All ladders shall be maintained in a safe condition. All ladders shall be inspected regularly, with the intervals between inspections being determined by use and exposure.    a. Facility yard - On or about January 15, 2014, employees were required to access sorting station platforms of the feed conveyor assembly using fixed ladders.  The fixed ladders were damaged and not maintained in a safe condition thereby exposing employees to fall hazards.

Facility yard - On or about January 15, 2014, the written energy control program did not include specific step by step procedures to control and lockout all energy sources for employees who performed service and or maintenance on such grinding equipment including, but not limited to, a pre-screener, conveyor belts and stationary mill.  Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
