29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   a. On or about January 15, 2013, an employee was not wearing a hard hat while other employees worked above him, exposing himself to being struck by falling building material.  The Transfomers Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 20 CFR 1926.100(a), which was contained in OSHA inspection number 315481929, citation number 1, item number 1 and was affirmed as a final order on 2/1/2012, with respect to a workplace located at 12298 Water Tupelo Street, Jacksonville, Florida, 32226.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems:   a. On or about January 15, 2014, on the roof trusses of a 6:12 pitch roof, an employee installing plywood sheathing was exposed to a 10-foot 8-inch fall hazard in that he was not protected by a fall protection system.  The Transformers Construction Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 943263, citation number 2, item number 1 and was affirmed as a final order on 11/15/2013, with respect to a workplace located at 95076 Lilac Drive Fernandina Beach, Florida, 32034.
