Transformers Construction services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 315481929, Serious citation number 01, item number 002 and was affirmed as a final order on February 1, 2012 with respect to a workplace located at 12298 Water Tupelo St. Jacksonville, Florida.   Transformers Construction services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 943263, Serious citation number 02, item number 001 and was affirmed as a final order on November 17, 2013 with respect to a workplace located at  95076 Lilac Dr. Fernandina Beach, Florida.   29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   a.   On or about January 17, 2014, on the 6:12 pitch roof, employees installing roof paper were exposed to a 9 feet fall hazard in that they were not protected by a fall protection system.

29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970   a.   On or about January 17, 2014, on the worksite, the portable fire extinguisher available when fueling the air compressor had not been maintained.
