29 CFR 1926.501(b)(1): Employee on a walking/working surface having an unprotected side or edge which is six feet or more above a lower level were not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system: a) 205 Easton Avenue, New Brunswick, NJ 08901. Employees were allowed to work on and from the top on newly poured concrete walls in excess of 16 feet to the ground below, removing concrete forms with no fall protection systems in place. Incident occurred on or about 01/16/14.

Employees were allowed to access a scaffold platform using a 12 foot Werner fiberglass A Frame ladder in the closed position when this ladder was not being used for purposes for which it was designed. Incident occurred on or about 01/16/14.
