29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary:     a) 307 Cook Avenue, Scotch Plains, NJ 07076. The employee (Foreman) was working in a trench/excavation when the spoils were placed at the edge of the trench with no retaining device in place to prevent materials or equipment from falling into excavation exposing him to crushing hazards.     Condition occurred on or about 01/16/14.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. Excavation was not made entirely in stable rock and exceeded 5 foot in depth.      a) 307 Cook Avenue, Scotch Plains, NJ 07076. The employee (Foreman) was working in a trench/excavation 9 foot deep, 3 foot wide and approximately 25 long with no protective systems in place, i.e. trench box.     Condition occurred on or about 01/16/14.
