29 CFR 1910.23(a)(10): The swing of a door and/or gate reduced the effective width of the platform to less than 20 inches:   (a) Shop #1 - Employees used a restroom at the top of a set of stairs, the platform in front of the door at the top of the stairs did not have at least 20 inches of clearance between the open swing of the door and the first step down, on or about January 16, 2014.   No abatement certification or documentation required.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one half hour:  (a) Shop Number 1 - Employer stored oxygen and acetylene cylinders together in the same area.  Employees were exposed to fire hazards on or about January 16, 2014.   No abatement certification or documentation required.

29 CFR 1904.32(b)(3): The person who certified the annual summary was not a company executive as defined in 29 CFR 1904.32(b)(4):  (a) OSHA 300A Annual Summary Of Work Related Injuries And Illnesses - The annual summary for calendar years 2013, 2012 and 2011 were signed by the company secretary who is not a company executive.  This was observed during log review on January 16, 2013.   No abatement certification or documentation required.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:   (a) Paint Shop - The employer did not provide the information in Appendix D to an employee who was authorized to voluntarily use a respirator during painting operations. The employee used the company provided respirator while he painted company vehicles in the company's paint shop, on or about January 16, 2014.   No abatement certification or documentation required.
