OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that, employees were exposed to, struck by and crush by hazards from loads being raised by hoist and lifting devices:   On or about January 13, 2014, and times prior thereto, the employer did not ensure that all hoist, hooks, chains, and below the hook lifting devices were frequently inspected, tested, and rated in the following locations:   1) Buffing Department Hoist #11 Dayton 2 Ton  2) Winding Department Hoist #16 Coffing 2 Ton  3) Winding Department Hoist #12 Jet 2 Ton  4) High Bay Crane #1 Yale 10 Ton  5) High Bay Crane #2 Coffing 10 Ton  6) Shear Area Hoist #9 Dayton 2 Ton  7) Radiator Seaming Area Hoist #18 Harrington 3 Ton 8) Radiator Seaming Area Hoist #19 Harrington 3 Ton  9) Marks Welding Area Hoist #21 Dayton 2 Ton 10) Jasons Area Hoist #20 Dayton 2 Ton 11) Shear Area Hoist #9 Dayton 2 Ton attached to Vacuum Lifter Safety Latch was tied back using a Zip Tie. 12) In the Winding Department , the Coffing 2 Ton Hoist used a spreader bar that was not labeled, rated or inspected. 13) In the Winding Department, the Jet 2 Ton Hoist used a spreader bar that was not labeled, rated, or inspected. 14) High Bay Crane #1 the Yale 10 Ton Crane used a spreader bar that was not labeled, rated, or inspected. 15) High Bay Crane #2 the Coffing 10 Ton Crane used a spreader bar that was not labeled, rated, or inspected. 16) High Bay Crane #2 the Coffing 10 Ton Crane used a dual spreader bar that was not labeled, rated, or inspected.
