Entire Facility: The employer's written lockout/tagout program did not include machine-specific procedures for various machines and equipment located in the establishment. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1910.19(d).

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed. Kruger Steel & Wire Inc. did not evaluate the lock out procedures periodically (at least once per year), thereby exposing employees to amputation hazards. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
