29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a) Straightaway Fabrications, LTD - Ashland, Ohio:  On or about January 14, 2014, the employer did not ensure that each employee who operated The Lodge and Shipley Co. manufactured (Model 120 PB-8, Serial # W166-5) mechanical press brake was protected from point of operation hazards.  Employees were exposed to crushing hazards, in that the wrist restraints were not adjusted correctly to prevent the operator from reaching into the point of operation during press brake operation.
