29 CFR 1926.451(c)(1)(ii): Guys, ties, and/or braces were not installed according to the scaffold manufacturer's recommendations or at the closest horizontal member to the 4:1 height and repeated vertically at locations of horizontal members every 20 feet (56.1 m) or less thereafter for scaffolds 3 feet ((0.91 m) wide or less, and every 26 feet (7.9 m) or less therafter for scaffolds greater than 3 feet (0.91m) wide. On or about January 14, 2014, at the above addressed jobsite the employer did not tie in the scaffold to prevent it from tipping. Employees working on the scaffold were exposed to a possible tip over hazard of the scaffold while performing stucco work. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level: On or about January 14, 2014, at the above addressed jobsite, employees were removing wrapping from a tubular welded frame scaffold, that was approximately 24 feet high and employees were exposed to injuries from falls due to the lack of guardrails. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
