29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:     On or about January 16, 2014, at Fire Station #5 under construction, the DeWalt 4 ½ inches Angle Grinder (Model #D28402  Serial #584992) used by employees to grind down reinforced steel did not have a guard. Employees were exposed to struck-by hazards.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:    On or about January 16, 2014, at Fire Station #5 under construction, the employer did not ensure that electrical equipment was free from recognized hazards that were likely to cause death or serious physical harm to employees:      1)   The insulation of a 50-foot yellow extension cord used to power the DeWalt 4 ½ inches Angle Grinder (Model #D28402 - Serial #584992) was cut through, exposing bare conductors.    2)   The insulation of a 25-foot red/black extension cord used to power the Craftsman Industrial 7 ¼ inches Worm Drive Saw (Serial #C381972) had strain relief broken, exposing bare conductors.    3)   The cord insulation of a 25-foot red/black extension cord used to power the Craftsman Industrial 7 ¼ inches Worm Drive Saw (Serial #C381972) had strain relief broken, exposing bare conductors.    4)   The 100-foot red/black extension cord (one of four extension cords connected together) used to power the Craftsman Industrial 7 ¼ inches Worm Drive Saw (Serial #C381972) had outer insulation damaged throughout the length of the cord.
