29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1):  Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports:     On or about January 16, 2014, at the Cross Point Church under construction, two employees were observed working on a two-tier scaffold that was not fully planked. On the second-tier, one employee was exposed to a fall greater than 10-feet.

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:      On or about January 16, 2014, at the Cross Point Church under construction, two employees working on a scaffold were observed climbing up and down from the scaffold without the use of a proper ladder. A six-foot A-Frame Werner ladder was used to climb up to the first tier of the scaffold, from there the employees used the sections of the frame to egress and access the second tier of the scaffold. The employees were exposed to slip and fall hazards from the lack of scaffold access.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed:  On or about January 16, 2014, at the Cross Point Church under construction, two employees working on a scaffold were observed climbing up and down from the scaffold and the roof without the use of a proper ladder. The employees were exposed to slip and fall hazards greater than 10-feet.    1)    A 6-foot A-Frame Werner ladder was used to climb up to the first tier of the scaffold; from there the employees used the sections of the frame to egress and access the second tier of the scaffold.   2)    A 12-foot A-frame Werner ladder leaned against the side of the building was used to access the approximately 12-feet above the ground.
