B)     American Trim, LLC, located at 999 West Grand Ave., Lima, OH 45801: On or about January 16, 2014, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the Edlund drill press (Model 2F15, Serial # 2F293). Employees did not isolate the electrical energy source in order to control hazards associated with rotating parts of the machine while performing activities such as, but not limited to, changing drill bits. Employees were exposed to injury from unexpected start-up and energy release.
