29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. A) American Trim, LLC, located at 999 West Grand Ave., Lima, OH 45801: On or about January 16, 2014, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the 3-roll mill machine (Rebuilt by Lehmann Mill, Inc., Model 14x30 DAY, Serial # 5678). Employees did not isolate the electrical energy source in order to control hazards associated with rotating parts of the machine while performing activities such as, but not limited to, cleaning the machine. Employees were exposed to injury from unexpected start-up and energy release. B) American Trim, LLC, located at 999 West Grand Ave., Lima, OH 45801: On or about January 16, 2014, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the Edlund drill press (Model 2F15, Serial # 2F293). Employees did not isolate the electrical energy source in order to control hazards associated with rotating parts of the machine while performing activities such as, but not limited to, changing drill bits. Employees were exposed to injury from unexpected start-up and energy release. C) American Trim, LLC, located at 999 West Grand Ave., Lima, OH 45801: On or about January 16, 2014, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the Kent USA horizontal lathe machine (Model TRL-1340G, Serial # 1340407159). Employees did not isolate the electrical energy source in order to control hazards associated with rotating parts of the machine while performing activities such as, but not limited to, changing set-up of the chuck.
