29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about January 14, 2014 and at times prior to employees were exposed to rotating parts, nip points, flying chips of metal and sparks in the following instances. a) MAZAK milling machine was missing a back guard. b) Vertical belt sander was missing guard on lower pulley.

On or about January 14, 2014 and at times prior to employees working in the machine shop were exposed to struck by and caught in hazards when operating grinder wheel equipment without guards in the following instances: a) De-Walt bench top mounted wheel grinder with wire brush located in machine shop. b) HAMMOND 4 grinder located in machine shop.
