29 CFR 1926.21(b)(6)(i): Employees required to enter into confined or enclosed spaces were not instructed as to the nature of the hazards involved, the necessary precautions to be taken, and in the use of protective and emergency equipment required. a) Gasoline pump pit - On or about January 16, 2014, an employee was working in a permit required confined space, which contained possibile atmospheric and flammable vapor hazards, without testing the space prior to entry or taking precautionary steps to mitigate the potential hazards.
