29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.   On or about January 16, 2014 at worksite 4628 Mercer Street in Kansas City, Missouri- Employee was using a pnematic nail gun to secure shingles to the roof without having any means of eye or face protective equipment.   Abatement certification for this item is not required.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems or personal fall arrest systems:  On or about January 16, 2014 at worksite 4628 Mercer Street in Kansas City, Missouri- Employees were performing roofing operations at an elevated height  without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.   Abatement certification for this item is not required.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:   On or about January 16, 2014 at worksite 4628 Mercer Street in Kansas City, Missouri- Employees were exposed to fall hazards from a stairway that had more than four risers that was not equipped with a stairrail system along each unprotected side or edge.   Abatement cerification for this item is not required.
